Prices will vary by your location and services that are provided. Please contact us for questions about LTA payment options. Lab requisitions/kit draw forms need to be filled out completely and emailed to LTA along with a copy of insurance card (front/back). This helps us insure we have proper equipment and prepare for your specimen collection.
No orders are scheduled or drawn without a physician order.
